Hospitalisation Expenses: Inpatient Treatment, Day Care procedure
AYUSH Treatment: Medical expenses incurred for inpatient care treatment under Ayurveda, Yoga and Naturopathy, Unani, Siddha and Homeopathy systems of medicines during each Policy Year up to the Sum Insured.
Pre & Post Hospitalisation: From doctor consultation or diagnostic tests leading to a hospitalisation or post-surgery recovery expenses, we cover them all.
Emergency Ground Ambulance: Expenses incurred on road Ambulances are subject to a maximum of ₹2000/—per hospitalisation.
Modern Treatment: Covers In-patient or as part of Day Care Treatment in a Hospital up to 50% of Sum Insured.
Cataract Treatment: This policy covers medical expenses incurred for treating Cataracts, subject to a limit of ₹40,000/—per eye in one policy year.

Cashless Claims
Step 1
Inform the claims team regarding hospital admission. Click here to send an email immediately to inform us about the incident, log in through the Self-i App, or call (022) 4890 3009 (Paid Number).
Step 2
Visit any network or non-network hospital, submit a pre-authorisation form, and get the desired treatment.
Step 3
Once the treatment is completed, submit all your medical reports, bills, etc., to the hospital. They will share these documents with us for verification.
Step 4
Upon verification, we will settle the applicable claim directly with the hospital on your behalf.
Reimbursement Claims
Step 1
Inform the claims team regarding hospital admission. Click here to send an email immediately to inform us about the incident, log in through the Self-i App, or call (022) 4890 3009 (Paid Number).
Step 2
Visit any network or non-network hospital and get the desired treatment.
Step 3
Once done, pay all the due medical bills and collect all payment receipts, invoices, and other necessary documents for submission.
Step 4
Submit your original scanned documents within 7 days from hospital discharge to initiate the reimbursement claim.
Step 5
We will verify your claim and medical documents and, upon approval, reimburse the applicable claim amount to your registered bank account.

Things to Keep in Mind While Filing a Claim
Terms and Conditions
For a policyholder with multiple disability health plans, you can only file a claim against any one of them.
The policy must be renewed within the 30-day grace period after expiry to avoid losing coverage.
Non-disclosure or submission of incorrect information can result in instant cancellation of your policy.
You must pay the deductibles or co-payment amount from your out-of-pocket expenses to initiate the claim settlement coverage for your medical requirements
You must make a nomination for your health insurance policy, who will receive the claim on your behalf in the event of your death.
The claim settlement amount will be paid based on the specific claim requests applicable to your chosen policy coverage and sum insured.
Responsibilities of a Policyholder/ Insured Person
Submit a completed cashless request form to the TPA or us for authorisation.
Provide all relevant medical information to the TPA or us if the pre-authorisation form request is denied.
Inform us regarding the medical treatment within 24 hours for emergencies or at least 48 hours before planned hospitalisation.
Provide any additional information or documentation required by us or the TPA to verify the claim request.
For reimbursement claims, submit all the necessary documentation to us or the TPA within 7 days after discharge.
Maintain a record of all your medical bills, payment receipts, etc.
Do not file any false claims or claims before the waiting period is over.
